Referral for Benefits Application Assistance

This form provides a referral to a Texas Community Partner Program service site for help with applying for or renewing benefits. Fill out this referral form for an email with a list of service providers to contact for an appointment. Referrals are NOT an application for benefits and have no bearing on eligibility determination.

Housing Stability Screening

An ACAM representative may reach out for more information. We encourage you to also contact Texas/United Way HELPLINE 2-1-1. Completing a screening or obtaining a referral does not guarantee financial assistance. 

Who is the form for? Someone who is: 

·         looking for rental/mortgage assistance (current or future) 

·         behind on monthly rent 

·         currently homeless 

·         current vacate or eviction notice 

Information you need to get started: 

·         Landlord contact information (if renting) 

·         Income in the past 30 days for all adults (18+) in the household 

·         Vacate or Eviction Notice (if applicable) 

Once you have the information in hand, the screen should take 10 minutes or less.
